Certified Birth and Death Certificates may be acquired through the Health Department either in person at the Plattsburg Office, or by mail.

$14 per record $11 for each additional copy |
The Health Department accepts cash, checks, and money orders.
If ordering vital records by mail, the application must be notarized. Records obtained in person do not require notary. A photo ID is required of any person obtaining a vital record.
